Objectives:An open label, randomised controlled trial was designed to evaluate the efficacy and safety of celecoxib (Celebrex, Pfizer) in the treatment of acute and chronic ulcerative colitis (ACUC).
Methods:A total of 3,849 patients were randomised to one of four active comparators (steroid-free, placebo-controlled, and open-label) in a 1:1 ratio to receive celecoxib (5 mg/day, twice weekly, for 12 weeks) or placebo for a duration of 24 weeks. Patients were randomly assigned to receive one of the three active comparators in a 1:1 ratio to a non-active comparator.
Results:At week 12, there was no statistically significant difference in the incidence of gastric ulceration (n=2,941; mean age, 45.4 ± 4.1 years) or gastrointestinal bleeding (n=1,742; mean age, 45.5 ± 3.4 years) among the three active comparators in the celecoxib-treated group. There was a statistically significant decrease in the incidence of gastric and duodenal ulcer in patients treated with celecoxib at week 12 (6.1% vs 5.0%, p<0.001) and week 24 (2.3% vs 5.9%, p=0.001).
Conclusions:A statistically significant reduction in the incidence of ulceration, gastric ulceration, and bleeding occurred in patients receiving celecoxib compared with placebo. This finding indicates the need for further trials to establish the efficacy and safety of celecoxib in these patients.
